This most recently occurred at 800 Highway 51, Lyons, Nebraska 68038 on May 5, 2017. There was no adequate documentation provided by the employer that showed specific procedures for locking out equipment, including but not limited to the Trunion Drum and the #2 Feeder Drag.

This most recently occurred at 800 Highway 51, Lyons, Nebraska 68038 on February 23, 2017. An employee who was performing maintenance activities on the North #2 Drum when their hand made contact with the drum and pinch point with the adjacent concrete barrier, causing an amputation.
