29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  On or about March 6th, 2017, at the jobsite located at 12305 SW 130 Street Miami, FL 33186, the employer did not perform annual audiograms for each employee exposed at or above an 8-hour time-weighted average of 85 decibels, such as but not limited to:  a. Noise levels of 66.37 % dose or an equivalent sound level of 87.0 dBA. The sampling was performed for 480 minutes.  b. Noise levels of 74.53 % dose or an equivalent sound level of 87.7 dBA. The sampling was performed for 480 minutes.
