29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 2916 McKinley Dr. Opelika, Alabama:  On or about March 7, 2017 and at times prior; the employer allowed his employee to perform framing activities while standing on top of the second floor wall without fall protection exposing his employee to a fall hazard of up to 18.10 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At 2916 McKinley Dr. Opelika, Alabama:  On or about March 7, 2017 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the second floor of the house and the rails of the ladder did not extend at least three feet above the landing area exposing his employees to a fall hazard of 12.5 feet to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a)  At 2916 McKinley Dr. Opelika, Alabama:  On or about March 7, 2017 and at times prior; the employer allowed his employee to use the top step of a six foot a-frame ladder to get off the top of the framed walls exposing his employees to a fall hazard of 6 feet to the floor below.
