29 CFR 1910.22(a)(1): The employer failed to provide a clean and sanitary condition for all places of employment, passageways, storerooms, service rooms, and walking-working surfaces. The employer is failing to ensure employees are protected from explosion hazards when conducting bagging operations in the "WD" area of the facility. Employees are bagging "Pro-Pep" animal feed supplements into 25KG bags. This was most recently documented at 4505 S. 76th Cir, Omaha, Nebraska on or about March 7th, 2017 when the employer had employees working in the area where excessive amounts of explosive dust were allowed to accumulate on the equipment, powered industrial trucks and electrical equipment in the facility.

Flexible cords were attached to the building surfaces, specifically, they were "zip tied" to the electrical conduit in this area. This was most recently documented at 4505 S. 76th Cir, Omaha, Nebraska on or about March 7th, 2017 when the employer had employees working in the area where extension cords are being attached to building surfaces and attached to multiple pieces of equipment in the facility.
