29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a. On or about March 07, 2017, employees engaged in construction activities at the above referenced worksite were working from scaffolding. Ground personnel were not wearing personal protective equipment to protect against falling / flying debris. Employees were thereby exposed to struck-by hazards.   No abatement certification or documentation is required for this item

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.   a. On or about March 07, 2017, each employee engaged in bricklaying activities from a scaffold were not protected from falling by a guardrail system at each of the working levels.  Employees were thereby exposed to fall hazards of approximately 18 feet.   No abatement certification or documentation is required for this item

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a. On or about March 07, 2017, at the above addressed jobsite, safety inspections were not conducted and hazards went un-identifying / un-mitigated, which could result in employee injuries. Employees were thereby exposed to falling / flying object, stuck-by, and fall hazards.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
