29 CFR 1910.27(a): Scaffolds.  Scaffolds used in general industry must meet the requirements in 29 CFR part 1926, subpart L (Scaffolds).    29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.     Site: in front of the building at 181 Varick Street, New York, NY 10014    On or about 3/8/2017,    a) The working platform on the scaffold was not fully planked between the front uprights and the guardrail supports.

29 CFR 1910.28(b)(12)(i): Scaffolds and rope descent systems. The employer did not ensure each employee on a scaffold is protected from falling in accordance 29 CFR part 1926, subpart L.      29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access.  Crossbraces shall not be used as a means of access.      Site: Front of building at 181 Varick Street, New York, NY 10014    On or about 3/8/2017    a) The employer did not provide a safe means of access to the scaffold platform used by employees, where this platform was more than 2 feet above the point of access.
