29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about March 08, 2017, and at times prior thereto, two workers were not protected by eye or face protection while using pneumatic hammers on the construction site, exposing the employees to the hazard of being struck by flying debris.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system. On or about March 3 2017, two workers installing the parapet on the commercial property under construction were not protected from falls over 6 feet.
