29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  a. On or about 11/12/2015, at Strand II, building II - personal fall protection was not attached to the basket anchor point by an employee while conducting wood finishing details from an aerial lift, exposing the worker to a 16 feet 10 inch fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. At building 2A, employees receiving roof trusses from a forklift were exposed to a 9 feet 1 inch and a 32 feet 8 inch fall hazards, in that, fall protection was not used, on or about 03/02/2017.

On or about 03/02/2017, at the job site - fall protection training certification was not provided for employees conducting various carpentry and wood framing tasks, and were exposed to 9 feet 1 inch, 16 feet 10 inch and 32 feet 8 inch fall hazards.
