29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a) On or about March 8, 2017 or prior there to, on Swedge Machine #2: The employer did not utilize energy control procedures on 230V powered electrical motor before employees removed a jam in the point of operation. Power was disengaged prior to maintenance activities by turning a rotational power switch without a lock out mechanism. NOTE: Abatement certification is required for this item.
