29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:     On or about March 9, 2017 and times prior thereto, at the jobsite, employees were exposed to a greater than 10 ft. fall hazard while working from a wooden pallet elevated on the forks of a Sky Trak 6036 telescopic forklift without approval from the manufacturer.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  On or about March 9, 2017 and times prior thereto, at the jobsite, employees were exposed to a greater than 10 ft. fall hazard while working from an elevated wooden pallet that was not secured to the forks of a Sky Trak 6036 telescopic forklift.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59):  On or about March 9, 2017, and times prior thereto, at the job site, the employer did not ensure that employees exposed to the hazards of silica and dust particles, while sawcutting brick and pouring bags of cement, were provided effective information and training on hazardous chemicals in their work area at the time of their initial training. This condition exposed employee(s) to inhalation hazards associated with exposure to airborne silica and dust particles.
