29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph. A. J & B Acoustical Inc. worksite located at 180 Shields Road, Avon Lake, Ohio: On or about February 22, 2017, a employee was operating a Skytrack 8042 and assisting an employee of Bay Mechanical & Electric Corp. in the removal of a metal grate when the incident occurred. The operator was not properly trained and evaluated in the operation of the rough terrain lift.
