29 CFR 1910.243(c)(5)(i): The spindle speed of the machine was not checked before mounting of the wheel to ensure that it did not exceed the maximum operating speed marked on the wheel: (a) TRAILER REPAIR BAY - Employee(s) used a 4.5 inch abrasive disc rated at 13,580 RPM to grind on a trailer ICC bumper using an air actuated sander with an 18,000 RPM maximum operating speed. The disc broke apart causing severe injuries to the employee, on or about March 8, 2107. No abatement certification or documentation required.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: (a) TRAILER REPAIR BAY - Employee(s) performed grinding operations on a trailer ICC bumper with an air actuated sander that was equipped with a 4.5 inch abrasive wheel, and the employer did not ensure that the employee(s) tool was equipped with a safety guard, which thereby exposed the employee(s) to severe injuries, on or about March 8, 2107. No abatement certification or documentation required.
