29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): Northeast corner of site: On or about March 09, 2017, and at times prior thereto, employees were exposed to a cave-in hazard while working in a trench that was over 5 feet in depth which was constructed with vertical walls and did not have a designed support system, shielding system or other protective system installed in the trench.
