29 CFR 1910.23(c)(8): The employer did not ensure that the top cap and the top step of a stepladder were not used as steps.      1) Jobsite: On or about 3/9/17, an employee used the top step and top cap of a stepladder to access a roof. 29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:      1) Jobsite:  On or about 3/9/17, an employee dismantling a shed roof for transportation purposes was approx. 8 feet above the ground level without the use of a fall protection system.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw was not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a) Jobsite - On or about 3/09/17, the employer did not ensure that the automatically adjusting hood which completely encloses the portion of the saw above the table and above the material being cut was in place prior to being used by employees thus exposing the employees to amputation/laceration hazards. An employee received a hand laceration when it came into contact with the rotating blade of the table saw.
