29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: On or about March 9th, 2017 and times prior to, at the above addressed job site, an employee performing grinding of screws using a Makita 1610 angle grinder (serial number 352808A) was exposed to struck-by and amputation hazards due to equipment being operated without a guard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59) On or about March 9th, 2017 at the above addressed jobsite, employees that perform railing installation were not provided effective training on hazards related to substances that contain chemicals such as but not limited to Quartz, Phenol and Titanium Dioxide, exposing them to chemical burns.
