29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:     The employer is failing to protect employees from fall hazards associated with working off of scaffolding that was not inspected by a competent scaffold erector prior to each use and after every move.  The most recent occurrence of this was evident during the inspection of the job site located at 501 N. Division Road, York, NE where the employer has employees exposed to fall hazards while working on scaffolding that was not inspected by the competent person prior to use or after it was been moved or altered erected.

29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   The employer is failing to protect employees from fall hazards associated with working off of scaffolding that was not fully planked on all working levels.  The most recent occurrence of this was evident during the inspection of the job site located at 501 N. Division Road, York, NE where the employer has employees exposed to fall hazards while installing a new horizontal auger and working off a single aluminum plank positioned across the top of the scaffold frame.

The most recent occurrence of this was evident during the inspection of the job site located at 501 N. Division Road, York, NE where the employer has employees exposed to fall hazards while they use the aluminum plank resting on the top level of the scaffold frame 10 feet 9 inches from the walking surface below; without the use of any form of fall protection.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards as required by paragraph (a)(1)-(5) of this section:   The employer is failing to train employees on the hazards associated with working off of a scaffold.  The most recent occurrence of this was evident during the inspection of the job site located at 501 N. Division Road, York, NE where the employer has inadequately trained employees exposed to fall hazards while working on scaffolding.  Instances demonstrating lack of required knowledge include but are not limited to:           i.    Employees lack of understanding the trigger height of 10 feet for fall protection while working off of a scaffold.        ii.    Employees lack of understanding that all working levels of a scaffold are required to be fully planked.       iii.    Employees lack of understanding that scaffold planking has a minimum required width of 18 inches.       iv.    Employees lack of understanding that frame scaffold is required to be assembled in accordance with its design and thus retaining (coder) pins are essential in preventing structural failure due to separation of components.        v.    Employees lack of understanding that every time a scaffold is moved it is required that a competent person inspect the scaffold to ensure there is no damage that occurred in the move.

Division Road, York, NE where the employer has inadequately trained employees exposed to fall hazards while working on scaffolding.
