WORKSHOP:  The employer had not established  and implemented step-by-step hazardous energy control procedures in machine specific for the employees during servicing and maintenance of equipment, such as (but not limited to) the  Frog Hopper Ride and additional rides at Stewarts Amusement.    On/or about March 8, 2017, the tower/boom that was hoisted up with two unsecured wooden blocks on the fence of the ride fell down onto an employee while the employee was working in a very narrow space between the frame of the ride and the tower/boom to replace of the cables , and the employee sustained very serious injuries form the incident.
