On or about March 6, 2017, located at Plant 8, employees were not provided with lockable devices for employees to lockout pneumatic ball valves for the installation and testing of a job made pneumatically-rammed seal between a dust collection bin and purge hopper, thereby exposing the employees to the unexpected release of 95-100 psi of stored energy.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a. On or about April 5, 2017, located at the Superblender packaging area, employees were exposed to the unexpected startup/release of stored energy of a KuKa robot or a shuttle bin while in the robotic cell performing setup activities. The employer failed to implement energy control application steps as the electrical disconnect was not isolated and LOTO devices were not applied to isolation devices [per the 1910.147(d)(4) requirements]. Instead a lock was placed on the locking mechanism of an interlock on a gate while an employee's key was left in the lock. The remaining applicable energy control elements involving dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6), were not implemented to protect employees from machine servicing hazards.

On or about March 6, 2017, located in Plant #8, employees were exposed to machine hazards associated with ingoing nip points while testing a newly installed pneumatically actuated seal that was located between the dust collector bin and purge collector. The employer failed to implement energy control application steps as LOTO devices were not applied to the pneumatic ball valve [per the 1910.147(d)(4) requirements] prior to testing. As a result, the remaining applicable energy control elements involving dissipation of residual energy [(d)(5)(I)] and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

On or about April 5, 2017, located throughout Plant #8, employees operated forklifts with spinner knobs attached to the forklift's steering wheel without the manufacturer's prior written approval. The spinner knobs affected the safe operation of the forklifts, thereby exposing the employees to the hazards associated with forklift operations.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about April 5, 2017, located at the end of conveyor the #3 Pug Mill conveyor in Plant #8, employees sorted brick material by hand where there was a 1-1/4 inch opening where the conveyor met the roller, thereby exposing the employees to an ingoing nip point. b. On or about April 5, 2017, located at the end of conveyor the #2 Pug Mill conveyor in Plant #8, employees sorted brick material by hand where there was a one inch opening where the conveyor met the roller, thereby exposing the employees to an ingoing nip point. c. On or about April 5, 2017, located at the levelators in Plant#8, there were pinch points created when the recessed platforms were raised while employees were standing or walking on, thereby exposing the employees to a caught in hazard.
