29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) On or about March 6, 2017 at 3440 Mhee McGeehee Rd, Montgomery Al: The employer failed to ensure an employee working from the JLG #16652 basket was connected to the basket with his lanyard which exposed him to the hazards of falls up to 30 feet.
