29 CFR 1910.243(c)(1): Abrasive wheels were used on portable grinders which were not provided with safety guards meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4). On March 14, 2017, employees were using hand held grinders in the fabrication area without safety guards. This condition exposed employee to struck-by hazards.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. On March 14, 2017, employees were using welding equipment which had spliced wires that were not spliced or joined together by brazing, welding, or soldering with a fusible metal or alloy. This condition exposed employees to electrical shock hazards.
