In the Stores area the employer is failing to ensure that the storage of material does not create a hazard - various diameters and lengths of metal are stored leaning against the wall unrestrained. These items include but are no not limited to two pieces of rectangular tube-like items measuring approximately 3" by 6" and 9'8" long. Employees are exposed to a struck-by hazard.

29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not constructed to prevent electrical shock: a. In Plant 1 the employer is failing to ensure that the pendant controls of bridge cranes are maintained in a manner that prevents electrical shock. The pendant on crane number 124, a 40 ton bridge crane located in the area known as the Hay Barn had been damaged and employees were exposed to a shock hazard. b. In Plant 2 the employer is failing to ensure that the pendant controls of bridge cranes are maintained in a manner that prevents electrical shock. The pendant on crane number 119, a 25 ton bridge crane located in the area known as the Radial Run-Out had been damaged and employees were exposed to a shock hazard.

In Plant 2 the employer is failing to ensure that Chain Sling Serial Number P2PH02 is inspected as required; the sling had not been inspected since Aug 13, 2015. Employees were exposed to a struck-by hazard. b. In Plant 2 the employer is failing to ensure that Chain Sling Serial Number 6016396-1-1 is inspected as required; the sling had not been inspected during the last two inspection cycles dated Aug 13, 2015 and Dec 22, 2016. c. In Plant 2 the employer is failing to ensure that Chain Sling Serial Number P2LB04 is inspected as required; the sling had not been inspected for over sixteen months between the last two inspection cycles dated Aug 13, 2015 and Dec 22, 2016. d. In Plant 2 the employer is failing to ensure that Chain Sling Serial Number 987011 is inspected as required; the sling had not been inspected during the last two inspections cycles dated Aug 13, 2015 and Dec 22, 2016.
