29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On or about 7 March 2017, and at times prior thereto, employees working within the production floor and bundling area were traversing over walkways that had either protruding objects, raised surfaces or depressions, exposing the employees to trip hazards.

On or about 7 March 2017, and at times prior thereto, two operators were each driving a forklift within the Bundling Area without having been retrained within 3 years, exposing other employees nearby to the hazard of being struck by the forklift.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. On or about 7 March 2017, and at times prior thereto, an employee manually cutting pieces of aluminum extruded strands would do so by stepping over a rotating shaft for the walking conveyor, exposing the employee to the hazard of being caught and pulled into the shaft.
