29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. On or about March 14, 2017, employees were exposed to amputation hazards from residual energy when two parts of an RP4 Roll Pack machine became jammed.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about March 6, 2017, employees were exposed to amputation hazards from hazardous energy when specific lockout/tagout procedures for the RP4 machine were not implemented.
