On or about 3/13/2017, Triad Manufacturing, Inc. had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training. Two employees were operating powered industrial trucks without completing the required training.

29 CFR 1910.178(p)(1): A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, and the truck was not taken out of service until it had been restored to safe operating condition. On or about 3/13/2017, an employee at Triad Manufacturing, Inc., was operating powered industrial truck in need of repair, defective, and unsafe. A TCM forklift, serial # TM2059, was missing a LPG locator/indexing pin. The forklift was unsafe and needed to be repaired, but was not removed from service until it was restored to safe operating condition.
