29 CFR 1910.178(q)(5): Parts of powered industrial trucks requiring replacement were not replaced with parts equivalent in safety with those used in the original design.    Employees were exposed to struck by hazards in the warehouse as the Hyster forklift had damaged tires with chunks of rubber missing.  The Hyster forklift also had half the seatbelt missing and the back up light was not working.  The forklift had not been taken out of service until it was in safe operating condition.
