29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) At the construction site of the Volcano Bay at the Universal Studios where employees operating and holding the hydraulic hose of a GV-ECO1400S Gilken Silent Piller were exposed to amputation and or crush hazards when the piler was in motion. Among others, some feasible and acceptable abatement methods to correct these hazards are: - Installation of guards around the perimeter of the roller - Install barricades around the silent piler - Install hose rollers (brackets on the piles) or a similar device as the one recommended by the manufacturer - The use of tag lines by the employees tending the hoses to keep them from getting damaged by the silent piler roller as it moves.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. (a) At the construction of the Volcano Bay at Universal Studios in Orlando, the employees operating the Silent Piler were not required to wear safety shoes. An employee suffered a toe amputation as observed on or about 03/03/2017.
