29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about 3/10/2017, at the above addressed jobsite, an employee was exposed to struck-by hazard while using a cement saw to cut cement block without using any eye protection.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  On or about 3/10/2017, at the above addressed jobsite, employees were exposed to a fall hazard while using the crossbraces to climb up a two-tier scaffold to erect a cement wall.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met (Construction Reference: 1926.59)(a)):  On or about 3/10/2017, at the above addressed jobsite, the employer did not develop or implement a written hazard communication program to address hazards related to substances that contain chemicals such as silica while employees were cutting cement blocks without any respiratory protection.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
