29 CFR 1910.23(c)(11): (11) Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface:  a) Exterior Front: Employees were exposed to a fall hazard in excess of 12 feet while using a portable ladder to access the roof that did not extend at least 3 feet about the landing, on or about 3/15/17.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  a) Roof: Employees engaged in removing snow from the roof were exposed to fall hazards in excess of 12 feet where they were not using a guard rail system, safety net, personal fall arrest system, or other feasible means of fall protection, on or about 3/15/2017.
