29 CFR 1910.119(h)(2)(iv): The employer did not develop and implement safe work practices consistent with paragraph (f)(4) of this section to control the entrance, presence and exit of contract employers and contract employees in the covered process areas: (a) On or about March 16, 2017, a chemical delivery driver was permitted unsupervised access to the anhydrous ammonia refrigeration machinery room, building 1A at 4001 State Street Milwaukee, Wisconsin 53208. This employee used a torch without a hot work permit.
