29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) in a safe and timely manner to assure safe operation: a) AU-18 Frigid Coil air handling unit (an evaporator) located in Shipping Dock; On or about March 10, 2017 and at times prior thereto, the employer exposed employees to anhydrous ammonia and struck-by hazards in that the employer failed to remove the AU-18 unit from service or provide interim protective measures until the unit could be replaced due to extreme corrosion on the mounts of the equipment which resulted in the unit falling and an ammonia release occurring.
