29 CFR 1910.25(c)(2): The employer did not ensure standard stairs have a maximum riser height of 9.5 inches (24 cm).    On or about 16 March 2017, and at times prior thereto, employees accessing the rooftop of the administrative building to the rooftop of the production facility did so with the use of a stairway in which the risers of such were not uniform and at least one riser had a height of approximately 18 inches, exposing employees using the stairs to the hazard of tripping and falling.

29 CFR 1910.25(c)(3): The employer did not ensure standard stairs have a minimum tread depth of 9.5 inches (24 cm).  On or about 16 March 2017, and at times prior thereto, employees accessing between the rooftops of the administrative building and the production facility did so with the use of a stairway in which the treads on one side were approximately 9 inches in depth but with approximately 5 inches of tread depth accessible, exposing the employees using the stairs to the hazard of tripping and falling.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  The following exceptions did apply: (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance;  and (7) the servicing or maintenance does not create hazards for other employees.    On or about 16 March 2017, and at times prior thereto, authorized employees performing service and maintenance activities on vacuum pumps without the training necessary to recognize applicable hazards, exposing employees to the hazard of being caught in the rotating belt and pulleys of the pump.

29 CFR 1910.28(b)(3)(v)(B): Each employee was not protected from falling through a hatchway and chute-floor hole by a removable guardrail system and toe-boards on not more than two sides of the hole and a fixed guardrail system on all other exposed sides.    On or about 16 March 2017, and at times prior thereto, employees would access the rooftop of the admin building using a fixed ladder and hatchway to the roof in which the hatch opening was not guarded with a railing system, exposing the employees to a fall hazard of approximately 13 feet 6 inches.

29 CFR 1910.28(b)(11)(ii): Stairways. The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails per Table D-2; stair width less than 44 inches (1.1 m) with two open sides required to have one stair rail system on each open side.  On or about 16 March 2017, and at times prior thereto, employees accessing the rooftops between the administrative building and the production facility did so with the use of a stairway that had two open sides but only one set of stair rails on the east side, exposing the employee to a fall hazard of approximately 48 inches or 4 feet.
