Safety of equipment shall be determined on the basis of the following considerations from damaged electrical insulation: At the residential construction site located at 5405 NW 94th Street, Kansas City, Missouri: Employees were exposed to electrical hazards. Employees were exposed to extension cords with damaged insulation. There were two extension cords with damaged insulation exposing interior wiring.

Employees were exposed to three extension cords with broken strain relief. Extension cords were used to run various equipment including an 4 inch angle grinder.
