29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the south side of the building, employees performing HVAC activities on the roof without fall protection were exposed to a fall hazard of approximately 18 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. Employees performing HVAC activities were accessing the roof area with a "Werner" ladder that did not extend at least 3 feet above the upper landing on the south side of the building and were exposed to fall hazards.
