29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 16, 2017 at or near 121 Maranes Circle, Maumelle, AR 72113, the general contractor did not ensure that subcontract employees operating nail guns were provided safety glasses to protect their eyes and or face from struck-by hazards that could cause physical harm.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about March 16, 2017 at or near 121 Maranes Circle, Maumelle, AR 72113, the general contractor did not ensure that subcontract employees were protected from fall from height hazards while removing and re-installing shingles on a 10/12 pitched roof.
