29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. a) 15 Battery Topside, on March 8, 2017 - the effectiveness of the energy control measures to stop flushing liquor from leaking from a gate valve with a broken packing gland follower was not verified prior to employees removing the broken pieces of the gland follower and cleaning the outside of the valve with a wire brush.
