29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:    (a)  KBP Coil Coaters @ 3600 E 44th Ave. Denver, CO: On or before March 15, 2017, the employer did not ensure that the side of the elevated work platform above the tower that overlooks the rollers and roller carriage was equipped with a mid-rail. This condition exposed employees to a fall hazard of 20 feet, and the potential for a fallen employee to be caught in the moving parts in the tower accumulator.    (b)  KBP Coil Coaters @ 3600 E 44th Ave. Denver, CO:  On or before March 15, 2017, the employer did not ensure that the opening in the tower platform used for ladder access was closed when employees were working on the platform. The platform was approximately 20 feet above ground level. This condition exposed employees to a fall hazard of approximately 20 feet.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a) KBP Coil Coaters @ 3600 E 44th Ave. Denver, CO:  On or before March 13, 2017, as part of injury/accident prevention responsibilities under its safety program, the employer did not ensure a periodic inspection was conducted of their energy control procedures on machines such as, but not limited to, the Entry Tower roll Accumulator.  On March 13, 2017, at the Paint Line Dept., two employees were seriously injured as a result of uncontrolled hazardous energy while re-threading a coil aluminum sheet through the Entry Tower Roll Accumulator. This condition exposed employees to crushing and caught-in-between hazards as a result of uncontrolled hazardous energy sources present while performing such activity.    Abatement note:  29 CFR 1910.147(c)(6)(ii) requires that the employer certify that periodic inspections of the energy control procedures had been performed.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:     (a) KBP Coil Coaters @ 3600 E 44th Ave. Denver, CO:  On or before March 13, 2017, as part of injury/accident prevention responsibilities under its safety program, the employer did not ensure employees were trained in the energy control procedures, as required under the standard, including the safe application, usage and removal of energy control devices.  On March 13, 2017, at the Paint Line Dept., two untrained employees were seriously injured as a result of uncontrolled hazardous energy while re-threading a coil aluminum sheet through the Entry Tower Roll Accumulator. This condition exposed employees to crushing and caught-in-between hazards as a result of uncontrolled hazardous energy sources present while performing such activity.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        (a)   K.B.P. Coil  Corters, Inc. @ 3600 East 44th Avenue, Denver, CO.  80216. On or before March 15, 2017, the employer did not ensure that adequate machine guarding was in place at the entry roller of the coil paint line process, where the coil is fed through to make the initial stitches.  This condition exposed employees to amputation, ingoing nip point, and crushing hazards.
