29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) On or about 3/15/2017 - at 15000 Jackson Trace Rd, Coker, AL, employees were exposed to fall hazards of approximately 12 feet above ground level while popping lines for shingles and securing the underlay on a new single family residence with a 10:12 roof pitch.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about 1/12/2017 - at 15000 Jackson Trace Rd, Coker, AL, employees were accessing a roof by using a 20 foot aluminum ladder that was not extended at least 3-feet above the upper landing surface, was not secured to a rigid support, and/or a grasping device was not provided. AMEX Roofing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 433031, citation number 1, item number 2 and was affirmed as a final order on 10/19/2012, with respect to a workplace located at 109 Dalton Lane, Muscle Shoals, AL, 35661.
