29 CFR 1910.22(a)(1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: a) Storage area, a metal flammable liquids storage cabinet which contained Luperox 101 had its door blocked open by a pallet of 55 gallon drums placed in the door's swing radius, and the aisle way to the cabinet was not kept clear, or about, April 5, 2017. b) Storage area, a wooden pallet and shrink wrap left over from the Luperox 101 shipment was left inside the metal flammable liquids storage cabinet, on or about, April 5, 2017. c) Storage area, the west wall and north wall of the perimeter around the metal flammable storage cabinet used to store Luperox 101, had empty plastic drums, empty cardboard carboys, and a storage rack system which prevented ready access to the exterior faces of the cabinet, on or about, April 5, 2017. ABATMENT CERTIFICATION REQUIRED
