Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about March 23, 2017 and at times prior, the product stored on the outer AB aisle racking system was unstable due to a bent and damaged bottom support upright, thereby exposing employees to a struck by hazard.      b. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   a. Located in the Boot Department: On or about March 11, 2017 and at times prior, employees were operating the roll and crimp machine number 243 to form three to fourteen inch diameter hvac duct collars and bodies. The approximately five eights inch gap between the rotating top roller and the metal guard created a pinch point, thereby exposing employees to a caught-in hazard.      b. Located in the Flowtite Cell: On or about March 20, 2017 and at times prior, employees were operating the roll and crimp machine number 2035 to form six to sixteen inch diameter hvac duct collars. The approximately one half inch gap between the rotating top roller and the metal guard created a pinch point, thereby exposing employees to a caught-in hazard.      c. Located in the Flowtite Cell: On or about March 20, 2017 and at times prior, employees were operating the roll and crimp machine number 1676 to form four to sixteen inch diameter hvac duct collars. The approximately three quarter inch gap between the rotating top roller and the metal guard created a pinch point, thereby exposing employees to a caught-in hazard.      d. Located in the 293 Cell: On or about March 23, 2017 and at times prior, employees were operating the roll and crimp machine number 1634 to form six inch diameter hvac duct collars. The approximately three quarter inch gap between the rotating top roller and the metal guard created a pinch point, thereby exposing employees to a caught-in hazard.      e. Located in the Boot Department: On or about March 23, 2017 and at times prior, employees were operating a Craftsman drill press to drill holes in stacks of sheet metal materials. The press had an unguarded rotating chuck and drill bit, thereby exposing employees to a caught-in hazard and struck-by flying chip hazard.

29 CFR 1910.219(i)(2): Couplings. Shaft couplings shall be so constructed as to present no hazard from bolts, nuts, setscrews, or revolving surfaces. Bolts, nuts, and setscrews will, however, be permitted where they are covered with safety sleeves or where they are used parallel with the shafting and are countersunk or else do not extend beyond the flange of the coupling.   a. Located in the Boot Department: On or about March 11, 2017 and at times prior, employees were operating the roll and crimp machine number 243 to form three to fourteen inch diameter hvac duct collars and bodies. The shaft coupling was approximately thirty-eight inches from the floor and was not adequately guarded with openings on the front and rear sides, thereby exposing employees to a caught-in hazard.      b. Located in the R&R Department: On or about March 20, 2017 and at times prior, employees operated the roll and crimp machine number 1760 to form four to fourteen inch diameter hvac duct collars and bodies. The shaft coupling was approximately thirty-eight inches from the floor and was not adequately guarded with an opening on the rear side, thereby exposing employees to a caught-in hazard.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.     a. Located in the 293 cell: On or about March 23, 2017 and at times prior, employees formed a crimped end on six inch hvac elbows using a crimping machine, machine number 343, with an unguarded point of operation, thereby exposing employees to a caught-in hazard.         Heating & Cooling Products was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii) which was contained in OSHA inspection number 1184296, citation number 1, item number 1, and was affirmed as a final order on December 1, 2016, with respect to a workplace located at 365 West Main Street in Byesville, Ohio 43723.
