On or about March 4, 2017, employees were exposed to electrical hazards from contact with an outlet that had been pulled out of its box and the employer did not determine if their work could bring any person into physical or electrical contact with the electric conductor.

Gabriela Almonte Olivos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1145284, citation number 1, item number 1b and was affirmed as a final order on June 17, 2016, with respect to a workplace located at 7210 N. Gilles Road in Edwards, Illinois 61528.     Gabriela Almonte Olivos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1157201, citation number 1, item number 1a and was affirmed as a final order on August 17, 2016, with respect to a workplace located at 5329 W. Flagstone in Peoria, Illinois 61615.
