29 CFR 1904.39(a)(2): The employer did not report, within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, to OSHA: (a) Facility, 1224 West Main Street, Monongahela, PA: On or about March 7, 2017 an employee sustained a work-related injury that result in an in-patient hospitalization and the employer did not report the hospitalization to the Pittsburgh OSHA office until March 16, 2017.
