Employees were exposed to cave-in hazards while working in an excavation 18 feet deep where the only means of egress required them to travel 42 feet laterally. 29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:    (a) On or about March 20, 2017, the employer did not ensure that protection was provided by placing and keeping excavated soil at least 2 feet from the edge of excavations, or by the use of retaining devices sufficient to prevent soil from falling or rolling into excavations, or by a combination of both if necessary. Employees were exposed to struck-by hazards in that a pile of loose and clumped soil, approximately 16 feet high, was placed less than 2 feet from the edge of the excavation without any retaining devices to prevent the soil from falling or rolling into the excavation. Employees were exposed to cave-in hazards while working in an excavation 18 feet deep, the excavation did not have an adequate sloping, benching or protective system in place in accordance with the above standard. Employees were exposed to cave-in hazards in that employees entered an excavation 18 feet deep where there was evidence of undermined walls, sloughing off of soil, stress cracks near the edge of the excavation, all of which are evidence of possible cave-ins. Excavation was not inspected daily by a competent person.
