29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.    At the construction site, an employee worked from a job-made work platform that was elevated by a tele-handler fork lift not specifically designed by the manufacturer exposing the employee to an unprotected fall hazard of approximately 20 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    At the construction site, employees engaged in roofing activities on a steep roof without fall protection were exposed to an unprotected fall hazard of approximately 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    At the construction site, employees engaged in roofing activities were accessing the roof from portable ladders that did not extend at least 3 feet above the landing exposing the employees to fall hazards of approximately 20 feet.
