29 CFR 1910.133(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there was a hazard from flying objects.  On or about 2/23/2017 in the machine shop the employer failed to ensure that employees wore protective eyewear where an employee was observed not wearing eye protection or using the chuck guard while performing cutting operations on the JET lathe.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  A. On or about 2/9/2017 in the machine shop, the employer failed to ensure the right work rest of the JET 8 inch bench grinder was adjusted exposing employees performing grinding operation to struck-by related injuries. The measurement between the frame and the stone was three eighths of an inch.  B. On or about 2/9/2017 in the machine shop, the employer failed to ensure the left work rest of the JET 8 inch bench grinder was adjusted exposing employees performing grinding operation to struck-by related injuries. The measurement between the frame and the stone was one quarter of an inch.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  A. On or about 2/9/2017 in the machine shop, the employer failed to ensure the right tongue guard of the JET 8 inch bench grinder was installed exposing employees performing grinding operation to struck-by related injuries. The measurement between the frame and the stone was one half inch.  B. On or about 2/9/2017 in the machine shop, the employer failed to ensure the left tongue guard of the JET 8 inch  bench grinder was adjusted exposing employees performing grinding operation to struck-by related injuries. The measurement between the frame and the stone was one half inch.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 P.S.I.:  On or about 2/9/2017 in the machine shop, the employer failed to insure that the compressed air that is used for cleaning equipment and personnel was reduced at or below 30 P.S.I. The measurement at output was 100 + P.S.I. The compressors for the plant air was set to between 113 and 119.9 P.S.I.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about 2/9/2017 in the machine shop, employees utilized tools and equipment receiving power from relocatable power taps (surge protectors/power strips).  The relocatable power taps including but not limited to the following were not used in accordance with their listing and labeling:  a. re-locatable power strip was used to power an overhead Comfort zone 120V 1500W heater .  b. re-locatable power strip was used to power a JET upright drill pump, light, auto feed, and orange extension cord used to power electrical hand grinders, cutting wheels, a bench grinder, and miscellaneous equipment.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about 2/9/2017 in the Machine shop, the employer failed to provide permanent wiring to employees performing work at the workbench in the middle of the shop where an orange extension cord was ran through the overhead joists and used to power tools and equipment, exposing them to fire hazards.
