29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  On or about 8 March 2017 and at times prior thereto, two employees without fall protection were performing painting activities on the roof of a house, exposing the employee to the hazard of falling approximately 10 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.  On or about 8 March 2017 and at times prior thereto, the foreman and an employee accessed the roof using an extension ladder positioned against the front of residence and traversed horizontally from the ladder to the roof, exposing the employees to the hazard of falling approximately 10 feet to the concrete sidewalk below.
