29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about March 21, 2017, employees were not provided hard hats while working around a crane with elevated loads, exposing employees to struck-by hazards. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 21, 2017, employees setting roof top units were exposed fall of over 25 feet while walking/working from the roof of a multi-residential property , the employer did not ensure the employees were protected from a fall. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
