29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about 10/1/2015 at 3501 Kumho Parkway Macon GA in the loading bay; employees were exposed to shock hazards where the Maxx Pro 42inch industrial floor fan provided by the host employer was available for use. Due to the yellow extension cord missing the grounding pin the outer casing would have become electrified at the moment it was plugged in.
