The workers were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee(s) provided with an alternative fall protection measures. The workers were exposed to a fall hazard.

The site foreman allowed workers to climb up on top of the wood deck, approximately 8 feet above ground level without fall protection and were exposed to a fall hazard.

The workers were up approximately five (5) rungs above ground level and were exposed to a fall hazard.
