29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE: 9750 Fall Ridge Trail, Sunset Hills, MO 63127, on or about 3/21/2017, workers were on top of a wood deck, approximately 8 feet above ground level, actively involved with the tear down of the deck.  The workers were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  JOB SITE: 9750 Fall Ridge Trail, Sunset Hills, MO 63127, on or about 3/22/2017, the employer failed to did not provide a training program for each employee potentially exposed to fall hazards, to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.   The site foreman allowed workers to climb up on top of the wood deck, approximately 8 feet above ground level without fall protection and were exposed to a fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  JOB SITE: 9750 Fall Ridge Trail, Sunset Hills, MO 63127, on or about 3/22/2017, workers were observed using a Werner A-Frame ladder designed to be used in the open and braced position, and it was used in the closed position and resting up against the wall.  The workers were up approximately five (5) rungs above ground level and were exposed to a fall hazard.
