(a) Location - 211 Mount Hope Street, Lowell, MA:  On or about March 21, 2017, employees replacing an existing roof on a residential home were not protected from falling while working at heights 18 feet above a lower level.

(a) Location - 211 Mount Hope Street, Lowell, MA:  On or about March 21, 2017, employees were accessing a roof on a residential home in which the ladder did not extend three feet beyond the roofs surface.
