29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about March 21, 2017 on a construction site where an employee was exposed to an electrical hazard when using an extension cord that had the outer insulation pulled back from the male end of the extension cord allowing strain to be applied to the terminal ends in the plug.
